What to Bring to Your Appointment:
- Insurance card(s)
- Photo ID
- List of current medications
- Any recent labwork or testing by other physicians that may be pertinent to your visit.
- Completed Patient History, Agreement of Payments and Benefits Agreement, Ownership in Facility, and the Consent for Purposes of Treatment Forms. (See below)
- Written referral from your primary care physician if your insurance plan requires that you obtain one. If you are not sure, you should check with your insurance provider ahead of time. (This applies to new AND established visits.)
When to Arrive for Your Appointment:
You should arrive at least 20 minutes prior to your appointment time if you have not already completed your forms. Please arrive at least 10 minutes ahead of time if your forms have already been completed. This will ensure that you spend as much of your appointment time as possible with your provider instead of filling out paperwork.
