What to Bring to Your Appointment:
- Insurance card(s) and Photo ID (If it has been over a year since your last visit or if any of your information has changed.)
- Completed Updated Medical History (See below)
- Written referral from your primary care physician if your insurance plan requires that you obtain one. If you are not sure, you should check with your insurance provider ahead of time. (This applies to new AND established visits.)
When to Arrive for Your Appointment:
You should arrive at least 10 minutes prior to your appointment time if you have not already completed your form. Please arrive at least 5 minutes ahead of time if your forms have already been completed. This will ensure that you spend as much of your appointment time as possible with your provider instead of filling out paperwork.
